29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): A) Express Services, Inc. DBA Express Employment Professionals, employing workers at America Manufacturing, Inc., at the site located at 2375 Dorr Street, Suite F, Toledo, OH 43607: On or about June 28, 2017, the employer did not evaluate the workplace and did not determine that temporary employees were exposed to hazards created during welding operations. Temporary employees were not required to use and were not provided with the flame resistant weld jackets while conducting activities such as, but not limited to, welding steel baskets. Temporary employees were exposed to injuries such as, but not limited to, burns. Express Services, Inc. DBA Express Employment Professionals was previously cited for a violation of this Occupational Safety and Health Standard (1910.132(d)(1)), which was contained in OSHA Inspection # 1154204, Citation number 1, Item number 1a, and was affirmed as a final order on July 8, 2016, with respect to the employers office located at 840 North Park Road, Wyomissing, PA 19610, and temporary employees exposed at Phytogenx, Inc. workplace located at 35 Thousand Oaks Blvd., Morgantown, PA 19543.
