29 CFR 1915.72(a)(1): Ladders with broken or missing rungs or steps, broken or split side rails, or other faulty or defective construction were used. When ladders with such defects are discovered, they shall be immediately withdrawn from service. Inspection of metal ladders shall include checking for corrosion of interiors of open end, hollow rungs. (a) HCI Industrial & Marine Coatings - 4350 NW Front Ave. Portland, Or. 97210 - barge 686 - On or about June 30th, 2017 and at times prior thereto, the CSHO observed a metal A-frame ladder with a broken rear horizontal support brace.
