29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Mold Shop: On or about June 28, 2017 and at times prior, the employer had an employee amputate his left index finger midway between the fingertip and the nail bed. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.
