29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. Employees engaged in operating, inspecting, testing, and maintaining activities on the covered process equipment of the Plant were exposed to chemical hazards associated with a catastrophic release of Ammonia in that the employer had not verified that all Piping and Instrumentation Diagrams (P&ID's) were up to date and accurate for use by Tyson employees and contractors for the process hazard analysis, line breaks, equipment/piping replacement/repairs, and etc. This was noted on 1"-HPL-C2 line connecting the E.L. Nickell 7'x18' High Pressure Receiver with an Evaporator in the EV-Maintenance Room 5110. The Tyson Foods was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.119(d)(3)(i)(B), which was contained in OSHA inspection number 1177103, citation number 1, item number 1 and was affirmed as a final order on 03/02/2017, with respect to a workplace located at 1001 East Stoddard Street, in Dexter, Missouri.
