29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Sawmill (Unscrambler and Stacker infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing lumber jams in and around the Unscrambler and Stacker infeed conveyor systems. b) Sawmill (Trimsaw infeed and outfeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing lumber jams at the Trimsaw infeed and outfeed conveyor systems. c) Sawmill (Sorter); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing lumber jams at the Sorter. d) Sawmill (Log Deck, Bucksaw including infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to unjamming wood debris, and cleaning in and around the Log Deck and Bucksaw infeed conveyor system. e) Planer Mill (Stacker and infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to cleaning and clearing jams at the Stacker and infeed conveyor systems. f) Planer Mill (Strapper); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing strapping jams at the Strapper. g) Planer Mill (Tilt Hoist and Planer infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to cleaning in and around the Tilt Hoist and Planer infeed conveyors.
