29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: Facility exterior, on top of tank trailer: The employer did not ensure that employees performing tank cleaning operations were provided a fall protection system: a) Employees were on the top of a tank trailer exposed to falls of approximately 10 feet without a personal fall arrest system. b) Employees were on top of the tank trailer exposed to falls of approximately 5.5 feet through the 20in diameter access hatch into the trailer. This condition was observed on/about 13 July 2017.
