Text For Citation: 02 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with confined space entry: Plant Wide - Employees were required to enter confined spaces such as, but not limited to, konus, core blender, surface blender, blender pit, and formers which exposed them to hazards such as, but not limited to, oxygen deficiency, formaldehyde, MDI, and combustion products, without full implementation of a confined space entry program. Employees enter the confined space for such purposes as cleanup, repair and maintenance. Prior to entering the confined space, essential precautions were not being taken to ensure rescue could occur if needed. Abatement Note: One feasible and acceptable method of abatement includes, but is not limited to: 1. Providing entrants with safety harnesses; 2. Having the emergency rescue equipment kept in close proximity to attendant at all confined spaces during entry. Your company was previously cited for a violation of this Occupational Safety and Health Standard which was contained in OSHA Inspection Number 109074674, Citation Number 1, item number 1, issued on 6/24/92.
