29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Jobsite, 208 & 210 Country Drive, on May 22, 2107, two employees were installing a new storm sewer in an unprotected trench that was approximately 9 feet deep when it collapsed which resulted in one fatality. The company Superintendent was present on the job site and was aware that the employees were working in the unprotected trench. R. A.
