29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities clearing fiber jams and changing out chain and belts on sprocket and pulleys. (a) New Albany Facility - On or about April 26, 2017 machine specific procedures were not developed, documented and used to prevent startup of fiber production machines while employees removed fibers from machines point of operation or when chains and belts were replaced on power transmission systems associated with production Lines A, B and C.
