29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): (a) Jobsite: On or about April 18 and 19, 2017, employees and the owner were erecting the tubular welded frame scaffolding that was about 15 inches from 120 v AC -240 v AC overhead electrical lines. This tubular welded frame scaffold was also only 4 1/2 feet from 23 KV phase to phase overhead electrical lines (13.2 KV phase to ground). None of these overhead electrical lines were insulated. In addition, the foreman and a laborer were working (removing and replacing the chimney mortar cap) from the very top of this tubular welded frame scaffold. When they were doing this they were both about 1 1/2 to 2 feet from the 23 kv (phase to phase, 13.2 kv phase to ground) energized overhead electric lines. (b) Jobsite: On or about April 20, 2017, employees were working from tubular welded frame scaffolding that was 15 inches from 120 vAC - 240 vAC overhead electrical lines. In addition, at least one employee (deceased) was working at the top of the scaffold and the employee was within 4 1/2 feet of the overhead energized, and not insulated 23 KV phase to phase (13.2 KV phase to ground).
