29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at ITC Manufacturing in Columbus, Ohio: a. On or about April 20, 2017, on the 2017 OSHA 300 Log, the employer did not provide all information required in column F such as the source of the injury. 1. Case 2, April 11, 2017, Column F stated, "Broke right forearm", which was insufficient to determine what the actual source of the injury was. 2. Case 3, April 18, 2017, Column F stated, "Laceration to left thumb", which was insufficient to determine what the actual source of the injury was. International Technical Coatings, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1904.29(b)(1)), which was contained in OSHA inspection number 1056337 citation number 2, item number 1, and was affirmed as a final order on 07/23/2015, with respect to a workplace located at 845 E. Markison Avenue, Columbus, OH 43215.
