29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): A) Maumee Assembly & Stamping, LLC, located at 920 Illinois Avenue, Maumee, OH 43537: On or about January 30, 2017, the employer did not properly guard the point of operation of the Verson TR-4 mechanical power press (Serial # 13558, 1200 Ton). The press was equipped with light curtains that were not operational. Employees were exposed to amputation hazards while operating the press. B) Maumee Assembly & Stamping, LLC, located at 920 Illinois Avenue, Maumee, OH 43537: On or about January 20, 2017, the employer did not properly guard the point of operation of the Verson BLK1 mechanical power press (Model # S4-1500-186-108T, Serial # 24680, 1500 Ton). The press was equipped with light curtains that were not operational. Employees were exposed to amputation hazards while operating the press.
