Text For Citation: 02 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling approximately 10 feet to the floor below: (a) On or about October 24, 1994, maintenance personnel climbed over the guardrails of the work platform and stepped over to line #3 Roe Air Conveyor to adjust the rails which guide the boxes product. The conveyors are approximately 1 1/2 feet wide. The employees walked between the rails in a crouched position to balance themselves while sliding their feet to keep from tripping over the rails. (b) On or about September 1, 1994, a maintenance person used metal mobile stairs to access line #3 and line #4 Roe Air Conveyors. The employee climbed over the rails of the mobile stairs to access the conveyors and then walked around the conveyors to adjust the guide rails. (c) On or about September 1, 1994, a maintenance employee stepped from the floor to the Bag Feed Station ledge, climbed and stood on the electrical control panel, climbed and walked on the framework of the machinery and lifted himself onto the Roe Air Conveyor at line #3 to perform change over functions while walking on the conveyor. The employee then stepped over to line #4 conveyor from line #3 air conveyor to finish the necessary change over procedures. Among other feasible methods, one feasible and acceptable method of abatement to correct this hazard is to ensure employees safe and proper usage of provided safe means of access and ensuring that employees' job duties are performed without employees accessing the equipment. General Mills, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 5(a)(1) which was contained in OSHA inspection number 122101595, Citation 1, Item Number 1, and Citation 2, Item Number 1, issued on June 10, 1994.
