Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Fabrication department, Littel coil line - on or about March 16, 2017, employees performing setup tasks (servicing) in the point-of-operation and associated danger zones were not required to apply energy controls in the following sequence prior to starting work: (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or tagout devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and de-energization of the machine or equipment have been accomplished. Operators assigned to perform setup (servicing) on energized equipment were exposed to machine caught-in hazards likely to result in serious injury because hazardous energy controls (lockout) were not implemented. Alternative measure, such as, but not limited to relying on a stopped cycle and/or program to prevent unexpected start-up on energized machines did not ensure equivalent safety. Other safeguards were not provided to prevent operators' access and/or disable hazardous operations start-up prior to exposure. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The J. L. Clark, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1052892, citation number 01, item number 001B and was affirmed as a final order on August 24, 2015, with respect to a workplace located at 923 23rd Ave., Rockford, IL 61104.
