Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Factory floor, main evacuation route, on or about 3/23/17: Two sets of double doors, both marked as an exit, each had the right side door locked at the top and bottom of the door. b) Main entrance to reception area, on or about 3/23/17: Two sets of double doors, both marked as an exit, each had the right side door locked at the top and bottom of the door.
