29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: (a) The employer did not ensure that an accident that resulted in an inpatient hospitalization of an employee was reported. On or about February 21, 2017 an accident occurred that resulted in serious injuries for an employee and the event was not reported until March 7, 2017.
