29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: On or about March 15, 2017, at the pool pump room area, employees are exposed to trip and fall hazards when walking by a 32 inches deep unprotected 8 feet (L) x 6 feet (W) floor hole. Employees access the area to open the backwash valve, to pick up and store chlorinating tablets container, and to add muriatic acid and calcium hypochlorite granules in the water pump area. NOTE 1: Sugar Bay Resort & Spa was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard [1910.23(a)(8)(i)] which was contained in OSHA Inspection Number 566738, Citation Number 1, Item Number 1, and was affirmed as a final order on 11/29/2012, with respect to a workplace located at 6500 Estate Smith Bay, St. Thomas, VI 00802. NOTE 2: Sugar Bay Resort & Spa was previously cited for a Failure to Abate (FTA) viiolation of this Occupational Safety and Health standard or its equivalent standard [1910.23(a)(8)(i)] which was contained in OSHA Inspection Number 566738, Citation Number 1, Item Number 1, and was affirmed as a final order on 09/16/2013, with respect to a workplace located at 6500 Estate Smith Bay, St. Thomas, VI 00802. NOTE 3: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
