29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours. On or about February 8, 2017, the employer had an employee hospitalized due to a fall in an unguarded maintenance pit and failed to report the hospitalization to OSHA within 24 hours.
