29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about February 22, 2017: a) The employer failed to report an in-hospitalization to the local area office of the Occupational Safety and Health Administration within the prescribed twenty four (24) hours. The incident and hospitalization occurred on February 17, 2017 at approximately 11:30 am.
