29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) On or about February 16, 2017, the employer did not provide adequate training to ensure that all employees threading plastic sheets at extrusion lines understood the purpose and function of the energy control program and acquired the knowledge and skills required for the safe application, usage, and removal of energy control devices, exposing employees to crush/amputation hazards. D&W Fine Pack LLC was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(7), which was contained in OSHA inspection number 1114406, citation number 1, item number 2c and was affirmed as a final order on August 5, 2016, with respect to a workplace located at 1100 Schwab Road, Hatfield, Pennsylvania, 19440. In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
