29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. On or about February 12, 2017, and at times prior to, in the Sulfur Recovery Unit at Houston Refining in Houston, TX, the employer did not ensure that the frequency of inspections and testing of process piping was performed in accordance with Recognized and Generally Accepted Good Engineering practices such as but not limited to API RP 570 (Piping Inspection Code: In-service Inspection, Rating, Repair, and Alteration of Piping Systems), when: a) The employer failed to inspect piping circuit 42-TG-1023-CAD (ISO439-0038-04), at the designated 5 year inspection interval (API RP 570, Table 2) at the following locations: 1. TML 1 2. TML 2 3. TML 3 4. TML 4 5. TML 5 6. TML 6 7. TML 7 8. TML 8 9. TML 9 10. TML 10 11. TML 11 12. TML 12 13. TML 13 14. TML 14 15. TML 15 16. TML 16 17. TML 17 b) After the piping was placed in service, subsequent measurements of 42-TG-1023-CAD (ISO439-0038-04), were not made at appropriate intervals to establish short and/or long term corrosion (API RP 570 Section 7.1.2), at the following locations: 1. TML 1 2. TML 3 3. TML 4 4. TML 5 5. TML 6 6. TML 8 7. TML 9 8. TML 10 9. TML 11 10. TML 12 11. TML 14 12. TML 15 13. TML 16 14. TML 17 c) Corrosion under insulation (CUI) inspection was not performed on piping circuit 42-TG-1023-CAD (ISO439-0038-04) to monitor corrosion (API RP 570 Section 6.4), at the following locations: 1.
