29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete. Polyvision does not ensure that the OSHA 300A Annual Summary of Injuries and Illnesses for 2016 includes all recordable injuries and recordable standard threshold shifts during 2016. Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that OSHA recordable injuries and recordable standard threshold shifts are included on the OSHA 300A Annual Summary of Injuries and Illnesses.
