29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work shift to employees when they were in their work area(s): The employer does not ensure copies of the required safety data sheets for each hazardous chemical, such as, but not limited to, sodium nitrite, potassium carbonate, M-43 and M-81 ball clay, Vansil W-20, borax, sodium glucoheptonate and magnesium carbonate are readily available to employees during the workshift in the employees work area exposing employees to hazards associated with contact with hazardous chemicals. Polyvision Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.1200(g)(8), which was contained in OSHA inspection number 1078680, Citation number 1, Item number 5b and was affirmed as a final order on February 3, 2016 with a final abatement date of February 4, 2016, with respect to a workplace located at 4301 N. Wood Drive, Okmulgee, Oklahoma 74447. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all safety data sheets are available. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: The employer does not provide effective information and training to employees who use chemicals such as, but not limited to, potassium hydroxide, crystalline silica, 3M Citrus Based Industrial Cleaner and Chemical Consultants Press Wash LV as well as inks used in the screen print process. Employees are exposed to contact with and inhalation hazards associated with chemical exposure. Polyvision Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 1078680, Citation number 1, Item number 6 and was affirmed as a final order on February 3, 2016 with a final abatement date on February 4, 2016, with respect to a workplace located at 4301 N. Wood Drive, Okmulgee, Oklahoma 74447. Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that effective information and training for hazard communication is provided to all employees. 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer does not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: The employer does not provide appropriate labels for containers of chemicals in the kitting and milling area. Employees are exposed to hazards associated with contact with hazardous chemicals such as, but not limited to, sodium nitrite, potassium carbonate, M-43 and M-81 ball clay, Vansil W-20, borax, sodium glucoheptonate and magnesium carbonate.
