29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about January 25, 2017, the employer did not record a work-related incident in the OSHA 300 logs. An employee sustained a back injury while operating a brake press and was taken to an Emergency Room for treatment.
