29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Larose Industries, LLC dba Cra-Z-Art - Fayette, Ohio: On or about February 14, 2017, the employer failed to report an amputation injury to an employee. On November 30, 2016, an employee sustained finger amputation injuries to four fingers; the employer did not report the amputation.
