29 CFR 1910.23(b)(9): Ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury: On or about 08 February 2017, and on prior dates, the employer failed to ensure that ladders used were inspected for damage. A ladder located in the maintenance shop was in service with visible damage. This exposed employees to a fall hazard. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
