Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specify procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) Production Area; On or about February 1, 2017, and at times prior, the employer exposed employees to amputation hazards, in that the employer failed to develop specific procedures for shutting down, isolating, and securing energy sources. Procedures were not developed and utilized when servicing and maintaining equipment such as, but not limited to the Fin-Kon Wicketer (Model 890-30-16 RHW, SN 07104) bag machine. Flexstar Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii)(B), which was contained in OSHA inspection number 1090761, citation number 1, item number 1, and was affirmed as a final order on March 30, 2016, with respect to a workplace located at 1902 Kimberly Park Dr., Dalton, Georgia 30720.
