29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: Location: Warehouse On or about 2/2/17, employees at the facility were exposed to rack collapse hazards and overhead struck-by hazards from racking not secured to their respective pallets, pallets not secured to racking, and damaged racking uprights in the following but not limited to areas; 1. Building #1, #2, and #3 - pallets are placed on top of racking on the top tier loaded with tires and are potentially overloading the Tier-Rack system. In addition the pallets are not secured to the racking below. 2. Building #1 - Rack section C3, aisle 4 had a bent right upright, 3. Building #1 - Rack section D10, aisle 5 had a bent right and left upright, 4. Building #1 - Rack section B5, row 4, had a bent in left upright, 5.
