29 CFR 1904.33(a): The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered. On or about January 19, 2017 and there prior, the employer failed to correctly document and save OSHA 300, 300A, and 301 logs for 2015 in the required OSHA log forms for at least five years.
