29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a. On or about February 7, 2017, located at the conveyor turnstyle southeast of the core baker, there was a set of stairs that did not have adequate guarding to protect employees walking through the area from falling into the turnstyle, thereby exposing the employees to a caught-between hazard. b. On or about February 7, 2017, located at the drag cleaner, there was an opening near where employees walked and where an operator would stand that did not prevent an employee from falling into the chute located under the drag cleaner, thereby exposing the employees to fall-related injuries. The Amsted Rail Company Inc. was previously cited for a violation of an equivalent occupational safety health standard 1910.23(c)(3), which was contained in OSHA inspection number 1030504, citation number 1, item number 1 and was affirmed as a final order on October 6, 2016, with respect to a workplace located at 2100 Griffin Wheel Drive, Bessemer, AL 35020.
