29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a. Paragon Tempered Glass, Inc. - Antwerp, Ohio: On or about January 3, 2017, the employer did not report a hospitalization of an employee following a work-related injury. On December 22, 2016, an employee sustained a serious laceration injury, resulting in hospitalization; the employer did not report the hospitalization.
