29 CFR 1910.27(c)(4): The employer did not install a fixed ladder so that the nearest permanent object in back of the ladder was not less than 7 inches, except when unavoidable obstructions wee encountered, the ladder was not installed with clearances as shown in figure D-3: Packaging Area, Backside of Pesto Room: On, or about, December 5, 2016, obstructions were located behind a fixed ladder that served as exit egress from the walk-on area.
