Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Machine operators who performed servicing and maintenance on the Ogden Glue Machine were not required to: 1. Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, 2. Turn off the shut down the machine, 3. Physically located and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, 4. Affix lockout devices to each energy isolating device, 5. Prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine or equipment have been accomplished. On November 10, 2016, a glue machine operator was exposed to a pinch point hazard while cleaning a clump of dry glue located next to the moving chain and sprocket for the bottom glue roller. Appalachian Wood Floors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1102208, citation number 1, item number 2b and was affirmed as a final order on December 11, 2015, with respect to a workplace located at 838 Campbell Avenue, Portsmouth, OH 45662.
