29 CFR 1904.4(a): The employer does not record each work-related fatality, injury or illness case that results in the general recording criteria on the OSHA Form 300 or equivalent. Location: Facility The violation was observed on 11/10/16, when the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016: 1) October 31st, 2016, nail gun injury: Nail impaled in cheek of employee requiring professional medical treatment. The employee lost one workday. To abate this violation, the employer must ensure that all recordable injuries are included on the OSHA Form 300 or equivalent. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance.
