29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): location: production room A on or about 11/7/16 a) The Matrix Mercury Fill Seal Machine, company #3,had interlocked guarding to prevent employees from coming into contact with the point of operation. The interlock was bypassed. Employees operated the machine with the doors of the guarding open, exposing them to the hazard of coming into contact with the points of operation. location: production room A on or about 11/7/16 b) The POD Machine, company #10 had interlocked guarding to prevent employees from coming into contact with the point of operation. The interlock was bypassed.
