29 CFR 1910.119(e)(5): The employer did not complete actions to resolve PHA recommendations as soon as possible: (a) In the tank car area, on or about March 17, 2017 and at times prior to, the employer failed to promptly address the recommendations made by the PHA team. Recommendations made during the September 21, 2015 PHA included installing pneumatic actuators at each rail station that had not been implemented.
