29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On October 31, 2016 an employee was seriously injured when struck-by a chain while working at the facility. The employee succumbed to his injuries on November 11, 2016. The employer failed to report the passing of the employee to the local area office.
