29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. a) Jobsite - The employer did not provide a safe means of access to the scaffolds on the form walls by the use of portable ladders, hook-on ladders or other acceptable means. Employees walked single planks from the grounds and sidewalk to access the top of the form walls from where they would then walk to their work area inside the form walls, approximately 12 feet above ground level. Observed on or about 10/18/16. The was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 994899, citation number 1, item number 1, and was affirmed as a final order on 4/1/15, with respect to a workplace located at 787 Swanson Street, Philadelphia, PA 19147.
