Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Plant - On or about 10/27/16 and at times prior thereto workers operated the MBI (Metallic Building Insulation) lamination machine, and the push bar (roll ejector) not guarded to protect workers from struck-by and crush hazards created by the sudden displacement of the push bar towards the stationary frame of the machine. Service Partners, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1147071, citation number 1, item number 3, issued on 06/08/2016 and affirmed as a final order on 07/05/2016 with respect to a workplace located at 7150 Business Park Drive, Suite 100, Houston, TX 77041. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
