29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: A.) GKN Sinter Metals - DuBois - On or about 10/19/26, the employer did not report a finger amputation/avulsion within 24 hours of the accident. The avulsion/amputation occurred on 10/19/16 at 1:00 am. The company did not report the fingertip avulsion/amputation until 10/24/16 at 4:00 pm.
