29 CFR 1904.39(a): An oral report of a work-related incident resulting in the death of any employee or the inpatient hospitalization of three (3) or more employees was not made within (8) hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration (OSHA) or to the OSHA toll-free central telephone number. 1-800-321-OSHA. Following the work-related death of an employee that occurred on September 13, 2016, at a jobsite located at 2727 Shelby Avenue, Unit I, Dallas, Texas 75219; the employer did not make an oral report to OSHA within eight (8) hours after the occurrence.
