29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 31, 2016, and at times prior thereto, employees were laying a sewer line for the Maple Park Utilities project in an excavation approximately 12 feet deep with vertical walls without the use of adequate cave-in protection, exposing employees to the hazards of cave-ins.
