29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: Employee was exposed to struck-by hazards in that the employer did not certify that one of the employees who operating a 16702-9 South-Worth Stack-N-Go battery powered motorized (electric) pallet jack - green machine; model #4439390; serial #71100-300-D4/130349 was trained, and evaluated in the safe operation of the pallet jack. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
