29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: a) Kunco Landscape, Inc. in Erie, PA. On or about August 22, 2016, the employer did not record workplace injuries or illnesses on the OSHA Form 300 for the calendar year 2016. Recordable cases include, but are not limited to, the workplace fatality that occurred on August 22, 2016 and the chemical burn that occurred on June 18, 2016.
