Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING
GENERAL 5A1 WORDING Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Crushing injuries, head injuries multiple fractures: The following conditions contributed to these hazards: 1.Lift # 1 had a safety stop catch missing. 2.Lift # 2 had a safety stop catch missing. 3.Lift # 3 had a safety stop catch missing. 4.Lift # 4's front hydraulic lift had a cylinder leak that made the lift unreliable when used in up position. 5.Lift # 4 had a safety stop latch that would not engage automatically and would not engage the day of the accident (rear cylinder ram). 6.There was no inspection program or shop maintenance being performed on the car and truck lifts as outlined by Midas' SHOP MAINTENANCE STANDARDS manual. 7.Employees were working under the car and truck lifts without any safety stop catches engaged and not using jack stands should the hydraulic lifts fail. The lifts were raised less than fully to perform specific work and employees were under and in the wheel wells. 8.Lift # 5 had a safety stop catch that would not engage unless manually manipulated. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO IMPLEMENT AN INSPECTION PROGRAM TO IDENTIFY MISSING SAFETY STOP CATCHES, SAFETY CATCHES NOT ENGAGING, HYDRAULIC LEAKS AND TO REPLACE OR REPAIR THEM. LIFTS BEING USED AND NOT EXTENDED FULLY SO THE SAFETY LATCHES WILL ENGAGE SHOULD HAVE CAR JACK STANDS PLACED UNDER THEM"
