29 CFR 1910.157(e)(2): Portable extinguishers or hose used in lieu thereof under paragraph (d)(3) of this section shall be visually inspected monthly. At the jobsite, the employer did not inspect and maintain portable fire extinguishers in the following instances: 1. A fire extinguisher (Serial Number: Z1-170667; Date of Inspection: 06/2016) at Dock Door S4, Column DW8 was found with expired tags and the viewing gauge revealed it to be uncharged. 2. A fire extinguisher (Model Number: 10HI SA80 ABC Model Number; Date of Inspection: 06/2016) at Dock Door S4, Column DW9) was found with expired tags and the viewing gauge revealed it to be uncharged. 3. On or about July 3, 2016, employee(s) in the Vehicle Maintenance facility were exposed to injury while responding to an incipient fire. The employee(s) attempted to extinguish a fire with the following fire extinguishers, which were found uncharged: a. Manufacturers Name: Amerex; Serial Number: ZA-833670; Model Number: B456 b. Manufacturers Name: Amerex; Serial Number: YL-72199; Model Number: B456 c. Manufacturers Name: Amerex; Serial Number: VK-573404;Model Number: B456 d. Manufacturers Name: Amerex; Serial Number: ZV-239619; Model Number: B456 c. Manufacturers Name: Amerex; Serial Number: SW-233160; Model Number: Unknown The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.157(c)(1), which was contained in OSHA inspection number 1144427, citation number 1 item number 1a and was affirmed as a final order on July 06, 2016, with respect to a workplace located at 7925 West Russell Boulevard, Las Vegas, Nevada 89113.
