29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not ensure all equipment used at the facility is provided with proper machine guarding, exposing employees to the hazard of amputation hazards. a) This violation was observed on or about July 19, 2016, where a Standard 200 ton hydraulic press brake was equipped with light curtains but were folded back/away from the point of operation and was inoperable. b) This violation was observed on or about July 19, 2016, where a Cincinnati hydraulic press brake was equipped with light curtains but was inoperable. Contract Manufacturer dba CM Trailers was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 316238658, citation number 1, item number 6, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446. Contract Manufacturer dba CM Truck Beds was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 316238732, citation number 1, item number 2, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure machine guarding is provided to protect operator(s) and other employees from hazard(s) created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
