29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: At the workplace located 907 Integram Drive, Pacific, Missouri the employer did not ensure the procedures for the control of hazardous energy for the Ras-4 Multibend-Center (Model No.: RAS 79.26-2) explicitly indicated who should maintain control of the key, nor does it instruct employee(s) to visually verify whether the authorized employee is performing maintenance on the machine before returning to normal operation. An authorized employee sustained a compound fracture to the left arm and a dislocated elbow when the machine was inadvertently restarted while maintenance was being performed. True Manufacturing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1138115, citation number 1, item number 1a and was affirmed as a final order on May 23, 2016, with respect to a workplace located at 2001 East Terra Lane, O'Fallon, Missouri 63366.
