29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Production Area / Peerless Cutting Machine #27: Lockout devices were not affixed by authorized employee performing tasks such as, but not limited to, clearing jams on the Peerless Cutting Machine, on or about 8/25/16. b) Production Area / Peerless Cutting Machine #13: Lockout devices were not affixed by authorized employee performing tasks such as, but not limited to, clearing jams on the Peerless Cutting Machine, on or about 8/25/16. c) Production Area / Peerless Cutting Machine #30: Lockout devices were not affixed by authorized employee performing tasks such as, but not limited to, clearing jams on the Peerless Cutting Machine, on or about 8/25/16. d) Production Area / Peerless Cutting Machine #16: Lockout devices were not affixed by authorized employee performing tasks such as, but not limited to, clearing jams on the Peerless Cutting Machine, on or about 5/9/16.
