29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) On or about June 9 - June 16, 2016 -- Buildings 2 and 6; The employer's written respirator program did not specify work specific procedures and elements for all respirators being used at the facility, such as but not limited to, 1/2 mask 3M respirators worn by painters and assemblers, positive air purifying respirators being worn by welders and air-line respirators worn by painters. The procedure for medical evaluation for all persons required to wear respirators had not been determined. Voluntary use of respirators did not specify only filtering face pieces (dust masks) or the requirement to provide Appendix "D" to voluntary filtering face-piece wearers. The respirator change schedule for persons wearing 1/2 mask respirators had not been determined. Procedures for cleaning and disinfecting the respirators had not been determined. Designated areas for respirator storage to prevent mis-shapened face-pieces for full and 1/2 mask tight fitting respirators had not been identified.
